    Kathleen Buchheit, MD

    Assistant Professor of Medicine

    Katie Buchheit, MD, is the assistant director of the AERD Center at the Brigham and Women’s Hospital (BWH) in Boston, Massachusetts. Her research focuses on understanding aspirin-exacerbated respiratory disease (AERD) and nasal polyposis. She is dedicated to investigating the causative mechanisms and exploring new treatments for these diseases. Dr. Buchheit is principal investigator of an NIH-funded study investigating the role of B cells and plasma cells in the pathogenesis of AERD and nasal polyps, as well as the mechanistic factors associated with nasal polyp recurrence. Clinically, she primarily focuses on the treatment of patients with chronic rhinosinusitis and AERD. During her allergy/immunology fellowship at BWH, she focused on the pathogenesis of nasal polyposis and AERD.

    Stella Lee, MD

    Director of the Brigham Sinus Center

    Stella Lee, MD is a fellowship-trained rhinologist and chief of rhinology at Brigham and Women’s Hospital (BWH)/Harvard Medical School in Boston, Massachusetts. She is a clinician scientist developing novel management strategies for the care of patients with inflammatory upper airway disease and is a leader in developing therapeutics for airway inflammation. Before joining BWH, Dr. Lee was the division chief for sino-nasal disorders and allergy in the Department of Otolaryngology at the University of Pittsburgh Medical Center for ten years. Her research interests include defining inflammatory endotypes of chronic rhinosinusitis, the impact of environmental pollution on airway inflammation, and the development of basic to translational approaches to solve chronic inflammation.

    Noam Cohen, MD, PhD

    Professor of Otorhinolaryngology – Head and Neck Surgery, Director of Rhinology Research

    Noam Cohen, MD, PhD is the Ralph Butler endowed professor of otorhinolaryngology – head and neck surgery and director of rhinology research at the Perelman School of Medicine at the University of Pennsylvania in Philadelphia. He is an adjunct member of the Monell Chemical Senses Center and a staff surgeon at the Philadelphia VA Medical Center. His research interests include host-microbe interactions in the upper airway, focusing on sinonasal innate defenses (airway taste receptors and mucociliary clearance), microbial biofilms, solitary chemosensory cells, and development of novel sinonasal topical antimicrobial therapies. He has authored more than 200 publications, given multiple presentations around the world, and has been principal investigator on NIH and VA grants, as well as industry-sponsored studies.

TARGET AUDIENCE

The intended audience for this program is ENTs, allergists, immunologists, specialty nurse practitioners and PAs, and other clinicians involved in the management of patients with nasal polyps.

STATEMENT OF NEED/PROGRAM OVERVIEW

As management strategies for nasal polyps (NPs) have evolved, patients whose NPs do not respond to initial treatment, who are ineligible for systemic corticosteroids, or whose NPs recur after initial surgery often undergo repeated surgeries, with high associated costs and complication risks. Clinicians—including ear, nose, and throat specialists (ENTs) and allergists—need to be aware of the latest strategies to personalize NP care and establish personalized treatment plans with the lowest risk of recurrence, toward improving patients’ overall quality of life, including their ability to breath and smell.

Join an expert panel of ENTs to explore the underlying pathways that lead to NPs and drive recurrent disease and treatment resistance. Panelists will also discuss the burdens faced by patients with NPs and evidence supporting the efficacy and safety of biologic agents for the treatment of refractory and recurrent NPs.

  • Discuss safety, efficacy, and impacts on bothersome symptoms of current antibody targeted therapies for patients with moderate-to-severe CRSwNP
  • Review patient-specific factors that help predict NP recurrence after standard treatment, including surgery

STATEMENT OF NEED/PROGRAM OVERVIEW

In a recent outcomes survey of patients with nasal polyps (NPs), 88% responded that their treatment journey had been extremely burdensome and that lack of communication with their care team was a barrier to proper diagnoses or changes in therapeutic approaches. With increased awareness of newer, more effective treatments, frontline clinicians who manage NPs and chronic rhinosinusitis with nasal polyposis (CRSwNP) can help alleviate patients’ disease burden related to inadequate symptom control and recurrent disease.

Join an expert panel of allergy and ENT specialists to review the latest understanding of NPs and CRSwNP, including severe NPs and those at high risk for recurrence. Panelists will clarify the place of functional endoscopic sinus surgery (FESS) and biologic agents in the treatment of CRSwNP. After participating in the education, clinician participants will recognize biologics available to treat NPs and understand which patient characteristics justify their use. Participating allergists ENTs will also be prepared to collaborate in the care of patients with NPs and include them in effective, shared decisions about treatment.

EDUCATIONAL OBJECTIVES

After completing this activity, the participant should be better able to:
  • Summarize the burden of disease and treatment for people with NPs, with a focus on outcomes that matter to patients, such as ability to smell, sleep, and be productive
  • Describe the T2 inflammatory relationships between NPs and common comorbidities, such as asthma, aspirin-exacerbated respiratory disease (AERD), and nonsteroidal anti-inflammatory drug-exacerbated respiratory disease (N-ERD)
  • Explain how enhanced knowledge of NP endotypes and T2 inflammatory pathways can guide treatment decisions
  • Review recent recommendations for the management of NPs with surgery and monoclonal antibodies, especially as they apply to the management of uncontrolled, severe NPs
